Before you begin on, recommended to making a backup of your system.
Oke, let's come on ;-)

If you want to upgrade from media cdrom, you can follow this step.
mount the disk to packages

mkdir /packages
mount /dev/cdrom /packages

The packages don't have to be on a cdrom, as alternative you should copy slackware directory , the one with the various package subdirectories on it, to some place like /root/slackware/ we'll use this dir for the following examples.

Put your machine to singel-user mode:
telinit 1 or init 1

Upgrade your package utilities and related tools:
upgradepkg /root/slackware/a/pkgtools-*.tgz
upgradepkg /root/slackware/a/tar-*.tgz
upgradepkg /root/slackware/a/xz-*.tgz
upgradepkg /root/slackware/a/findutils-*.txz

Upgrade your glibc shared libraries.
upgradepkg /root/slackware/a/glibc-solibs-*.t?z
